Consultation Visits (New Patients)
For your initial consultation, please plan to be in the office for approximately 2 hours from check-in to check-out.
During this visit, the doctor will:

Follow-Up Visits
For follow-up appointments, please plan to be in the office for about 1 hour.
If your visit requires additional forms, paperwork, or medical documentation, the appointment may take up to 90 minutes.

We accept a variety of insurance plans. Please contact our office or check with your insurance provider to confirm coverage.
Do you take self-referrals or second opinions?
No, we operate on a referral-only basis and do not accept self-referrals or requests for second opinions.
Do you treat workers’ compensation cases?
No, we do not handle workers’ compensation cases at this time.
What is your cancellation policy?
We kindly request that you provide at least 24 hours’ notice if you need to cancel or reschedule your appointment.
Do you treat pediatric patients?
No, we primarily treat adult patients. Please consult a pediatric specialist for care tailored to children.

How do I schedule an appointment?
Appointments are scheduled by referral only. Please have your primary care provider fax the necessary documents to our office. Once received, our team will contact you to arrange an appointment.
What should I bring to my first appointment?
Please bring:
• A valid photo ID.
• Your insurance card.
• Any relevant medical records or test results your referring physician has not already sent.
Do you offer telemedicine appointments?
Telemedicine appointments may be available for follow-up visits. Please contact our office for more information.
PRESCIPRTIONS/PAYMENTS
Can I get prescriptions refilled through your office?
Yes, prescription refills can be requested during your appointment or by contacting our office. Please allow up to 48 hours for processing.
Do you offer cash payment options?
No, we do not accept cash payments. Services must be covered through insurance plans we accept.
